Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please be advised that we try to obtain information about your insurance coverage before your physical therapy sessions.  However, it is your responsibility to check with your insurance company and be aware of all deductible, co-pays, co-insurance, visit limitations, prior authorization requirements and financial caps in advance. 

We participate with most insurance companies including Medicare, all Medicare Advantage programs, Blue Cross Blue Shield, Blue Care Network, Priority Health, PHP, Cofinity, and all Auto and Workman Compensation Companies.

Here is a list of questions to ask your insurance company about your physical therapy coverage:

Do I have physical therapy insurance benefits?
What is my deductible and has it been met?
How many sessions per year does my health insurance cover?
What is the coverage amount per physical therapy session?
Is approval required from my primary care physician?

Billing
Effective Wednesday, March 4, 2014, our billing services have moved to Clinicient, an outside billing service. The account statements you recieve in the mail with balances due will come directly from Clinicient. Questions about your statements should be directed to Clinicient by calling toll free 1-866-451-7596.

Payment

Payments are to be paid to our office at 3751 South State Road, Ionia, MI 48846. Cash, check and all major credit cards accepted for payment.
